I acknowledge that there is an element of risk and I hereby release and forever discharge Paws For Fun Canine Centre Ltd and their staff, from all actions, claims and demands whatsoever for any damage, loss or injury to person, property or animal, which may be sustained in consequence of the obedience/agility course taken.  I certify that to the best of my knowledge that my dog is in good health and has not been in contact with any infectious diseases.  I have read and agree to abide by the rules and regulations of Paws For Fun Canine Centre Ltd.
